Most insurance-based sessions are limited to 30-45 minutes, and the physical therapist might only spend 15-20 minutes of that time actually working with you. The rest? You’re doing exercises while they rush between other patients. It’s like trying to have a meaningful conversation in an elevator.

Insurance companies love protocols. Knee pain? Here’s the knee protocol. Back pain? Here’s the back protocol. But here’s the thing—your body isn’t a protocol. What’s causing your knee pain might actually be coming from your hip, your foot, or the way you’ve been compensating for an old ankle injury.

I’ve seen too many people who were just starting to make progress when their insurance said “no more visits.” It’s heartbreaking because we both know they need more time, but the system says they’re “done.”

That’s why I spend at least two hours in our first visit really understanding what’s going on with you… Your medical history, how this pain is affecting your daily life, what you’ve tried before, what activities you’d like to return to. And we do this in your home, where I can actually see how you move in your real environment.

Instead of rushing through a 30-minute assessment, I take the time to understand your complete picture. We sit down, we talk, I observe how you move in your actual environment, and I perform the kind of thorough examination that gets to the root of what’s causing your pain.

You don’t have to drive somewhere when you’re hurting. I bring all the equipment we’ll need directly to you. I can assess things like how you get in and out of bed, how you navigate your stairs, and how you move through your daily activities. I can show you exactly how to modify activities and set up exercises so there are no confusions.

I’m not interested in just managing your symptoms. I want to find out exactly what’s causing your pain and fix that. It’s like being a detective—I keep digging until I find the real culprit, not just the obvious suspect.

I’m a Board Certified Orthopedic Clinical Specialist, which means I’ve passed a six-hour exam that tests advanced knowledge in diagnosing and treating complex orthopedic conditions. I’ve also got specialized training in things like the McKenzie Method for back pain and custom orthotic fabrication.

While we don’t bill insurance directly, many patients submit their receipts to their insurance for potential reimbursement. We can provide you with all the documentation you need. The bigger question is: would you rather deal with insurance limitations or get the care you actually need?

If you have space to put a yoga mat down then we will have enough room. In confined spaces we can move a coffee table against the wall – that’s enough room. We will expect you to do a daily ‘home exercise program’, and we will show you exactly how and where to set them up in your home.

Most patients see significant improvement within 8 visits. After the first visit, clients typically purchase an 8-visit package. After the 8th visit, some clients purchase a second package. If someone needs a third package, either we are doing our job wrong or they have a crush on us! But seriously, we work efficiently because we’re addressing the root cause, not just managing symptoms.

Here’s the thing: if we can’t help you improve your condition we will know early on in the process and we will help you find someone who can. That’s part of the Kokua philosophy.
