Cash Pay Physical Therapy in Northern Vermont
How Our Cash-Pay Mobile Physical Therapy Works

Why Insurance-Based PT Often Leaves You Frustrated
The reality is that our current healthcare model doesn’t feel the same way about movement and recovery as we do at Kokua. When I was working in insurance-based clinics, I saw firsthand how these limitations negatively affect patient outcomes:
❌ Time Constraints Kill Results
Most insurance-based sessions are limited to 30-45 minutes, and the physical therapist might only spend 15-20 minutes of that time actually working with you. The rest? You’re doing exercises while they rush between other patients. It’s like trying to have a meaningful conversation in an elevator.
❌ Cookie-Cutter Approaches Don't Work
Insurance companies love protocols. Knee pain? Here’s the knee protocol. Back pain? Here’s the back protocol. But here’s the thing—your body isn’t a protocol. What’s causing your knee pain might actually be coming from your hip, your foot, or the way you’ve been compensating for an old ankle injury.
❌ Visit Limits End Care, Not Recovery
I’ve seen too many people who were just starting to make progress when their insurance said “no more visits.” It’s heartbreaking because we both know they need more time, but the system says they’re “done.”
❌ Authorization Delays Interrupt Healing
Sometimes we have to wait days or even weeks for insurance authorization while you’re sitting at home in pain. From a continuity of care standpoint, this is not ideal. Your body doesn’t pause its healing process while we wait for paperwork.
This is why I believe so strongly that cash-pay physical therapy isn’t just a different payment method—it’s a completely different approach to your health and recovery.

Experience Physical Therapy as It Should Be
You deserve a thorough assessment. That’s why I spend at least two hours in our first visit really understanding what’s going on with you… Your medical history, how this pain is affecting your daily life, what you’ve tried before, what activities you’d like to return to. And we do this in your home, where I can actually see how you move in your real environment.
✅ 2-Hour Comprehensive Evaluations
Instead of rushing through a 30-minute assessment, I take the time to understand your complete picture. We sit down, we talk, I observe how you move in your actual environment, and I perform the kind of thorough examination that gets to the root of what’s causing your pain.
✅ In-Home Convenience
You don’t have to drive somewhere when you’re hurting. I bring all the equipment we’ll need directly to you. I can assess things like how you get in and out of bed, how you navigate your stairs, and how you move through your daily activities. I can show you exactly how to modify activities and set up exercises so there are no confusions.
✅ Root Cause Focus
I’m not interested in just managing your symptoms. I want to find out exactly what’s causing your pain and fix that. It’s like being a detective—I keep digging until I find the real culprit, not just the obvious suspect.
✅ Board-Certified Expertise
I’m a Board Certified Orthopedic Clinical Specialist, which means I’ve passed a six-hour exam that tests advanced knowledge in diagnosing and treating complex orthopedic conditions. I’ve also got specialized training in things like the McKenzie Method for back pain and custom orthotic fabrication.
✅ Unlimited Time for Healing
Your treatment isn’t limited by insurance authorizations. We work together until you’re actually better, not until some insurance company decides you’ve had enough.
✅ Real Results
Most of my patients see significant improvement within 8-12 sessions. Not months of slow progress, but real, measurable change in how you feel and how you move.

The Real Cost of "Cheaper" Insurance-Based PT
I know what you’re thinking: “This sounds great, but it’s probably expensive.”
The Value Equation
VALUE = (quality of care + client experience + time with expert clinician + results) / cost.
Take Chris, who had bilateral knee pain and low back pain. After multiple failed treatments attempts with his physician, naturopath, PRP injections, and insurance based PT (that made him worse), he invested in 8 sessions with Kokua PT. The result? Seven-mile hike, pain-free.
Package Pricing
- 4-Session Package: $800
- 8-Session Package: $1,500 (most popular)
- Individual Sessions: $220
Many patients are pain-free within 8 visits. Compare that to insurance-based PT where you might exhaust yearly visits without results.

What do our PATIENTS say about us?
Your Questions Answered
❓ "Can I use my insurance for reimbursement?"
While we don’t bill insurance directly, many patients submit their receipts to their insurance for potential reimbursement. We can provide you with all the documentation you need. The bigger question is: would you rather deal with insurance limitations or get the care you actually need?
❓ "How much space do I need at home for treatment?"
If you have space to put a yoga mat down then we will have enough room. In confined spaces we can move a coffee table against the wall – that’s enough room. We will expect you to do a daily ‘home exercise program’, and we will show you exactly how and where to set them up in your home.
❓ "How many sessions will I need?"
Most patients see significant improvement within 8 visits. After the first visit, clients typically purchase an 8-visit package. After the 8th visit, some clients purchase a second package. If someone needs a third package, either we are doing our job wrong or they have a crush on us! But seriously, we work efficiently because we’re addressing the root cause, not just managing symptoms.
❓ "What if this approach doesn't work for me?"
Here’s the thing: if we can’t help you improve your condition we will know early on in the process and we will help you find someone who can. That’s part of the Kokua philosophy.
We’ve been doing this for years, and our approach works because we take the time to get the diagnosis right. So, in the spirit of ha’aha’a (humility), if our approach is not right for you we will refer you to a medical provider who can help.
❓ "How is this different from other cash-pay physical therapists?"
If you’ve made it this far we’re pretty sure you understand what makes Kokua different. We care about you and we are passionate about what we do.
On top of that, Teddy’s advanced certifications, unique professional experience, and our Aloha spirit set us apart.
